The student health affirmation log must be completed by students on campus every day. For quick access, this form can be completed on iOS and Android Mobile devices using Microsoft Power Apps.
Go to the app store on your device and download Power Apps developed by Microsoft.
When the app is installed, sign in with your smcvt.edu e-mail account.
Enter your password when prompted and Sign In.
** If you forget your e-mail password: you must contact the IT helpdesk at 654-2020 or ithelp@smcvt.edu to reset it. You cannot reset your e-mail password yourself. **
Once logged in, tap the down arrow and select All Apps.
Tap on the icon for Student Health Log.
You must Allow the connection to SharePoint to continue. (App will not work if you click Don’t Allow.)
Now you can complete the affirmation form.
Mobile setup is complete. Your device will remember credentials so you can access the Student Health Log with a few quick taps.
